Creating a Game Plan

The Game Plan is a structured, guided process that helps you clarify what matters most and translate those priorities into guidance for future decisions. 

Through four conversations and a final family meeting, we explore your values, your concerns, and the situations that may arise in the years ahead. The result is a written Game Plan that helps both you and your loved ones approach decisions with clarity and shared understanding.

Game Plans for Individuals With Dementia

When someone is living with dementia, planning early can be especially meaningful.

A Dementia Game Plan offers the opportunity to clarify values and priorities while the individual can still participate in the conversation. This creates a framework that can guide families and Health Care Agents as care needs change over time.

When someone can no longer participate fully, I work with their Health Care Agent and trusted loved ones to create a Game Plan based on what they understand about the person’s values and wishes. The plan becomes a steady source of guidance, helping prevent confusion or conflict and supporting thoughtful decisions as circumstances evolve.

Every person and every family is different, so the process remains flexible. The goal is always the same: a clear, values-based framework that supports good decisions and stronger communication in the years ahead.
